Dental Health Advantage

At Morgan Dental Care of the Portland Maine area, we are committed to helping you maintain your wellness.  When we entered these tight economic times we saw many people having a harder time funding their healthcare.  We know it can be tempting to postpone or skip preventive dental care during a time like this, but we also know that doing so will jeopardize your health.  So we have instituted a plan called Dental Health Advantage to help you continue regular preventive care.

It’s a way to make your preventive care more effective and more affordable.

It rewards you financially for keeping up your optimal maintenance schedule for your dental health!

All you have to do to earn up to $40 credit per preventive visit, is what you already know you should do:

• Keep your regularly scheduled appointments
• Stick to the recommended interval between visits
• Be consistent with your care
• Keep your account current

You will be rewarded:

• With health maintained at a higher level
• With credits you can use for any dental service or product we provide

So how does it work?  After enrolling, each time you come for your appointment with your hygienist on-time and at the prescribed interval, you will receive a credit on your account.  For the first visit, the credit will be $10.  The next time it will increase to $20.  After that, your credit will go to $30 and then to $40.  From then on, you will receive a $40 credit each time you come!

This is real money credited to your account that you can use for any service or product we provide!

The fine print:
To receive a credit, your account as well as accounts of your dependents must be paid in full on or before the day of service.
Credits are not payable in cash:  they may only be used for a service or product that we provide.
Your appointment must be within 30 days of the interval prescribed by Morgan Dental Care
You must keep all other appointments scheduled with Morgan Dental care whether or not they qualify for credit under this plan
If you do not fulfill these conditions, you will still be enrolled, and keep any previously earned credits on your account, but no credit is given for any appointment where the conditions are not met.  The next appointment where you do meet the conditions, the earned credit will be $10.
There are no exceptions to the rules:  for example an appointment missed due to illness

Enrollment fee is $10, equal to your first credit, so enrolling costs you nothing.  Enrollment is valid for as long as Morgan Dental Care, your Portland Maine dentist, at its sole discretion, chooses to participate in DHA and you remain a patient of record in this practice.

Dental Health Advantage Agreement

I understand that Dental Health Advantage is an agreement between myself and Morgan Dental Care designed to maintain my dental health at its highest level, while making my preventive care more affordable.

I agree to:

1. Keep my appointments as scheduled and on time
2. Give Morgan Dental Care at least 2 business days’ notice if it becomes necessary to change an appointment
3. Schedule my appointments at the interval recommended to me by Morgan Dental Care
4. Pay for my preventive care before or on the day of service and keep my account current

In return, each time I keep these four agreements, Morgan Dental Care will credit my account:

$10 for the first preventive appointment
$20 for the next consecutive preventive appointment
$30 for the third
$40 for the fourth and all subsequent consecutive preventive appointments

I understand that I will not earn credit any time I don’t keep the four agreements and I start over at $10 the next time I keep them

I understand that I will earn credit according to the above schedule every time I keep the four agreements and that there are no exceptions to these rules for earning these credits.
